Regarding extramedullary hematopoiesis, it's my understanding that minimal ( i . e . negligible ) hematopoiesis occurs in lymphoid tissue even in healthy adults.

Bone marrow biopsies are performed to ensure the disease is localised; and in SPB or extramedullary plasmacytoma there will not be an increase of monoclonal plasma cells.
